Clinical History 19

Seventy-two year old gentleman with severe coronary artery disease and severe left ventricular systolic dysfunction who presents for a follow-up cardiac evaluation. Co-morbid conditions include peripheral vascular disease, insulin requiring diabetes mellitus and hypertension. His medications at the time of this electrocardiogram included captopril, isosorbide mononitrate, furosemide, warfarin, topical nitroglycerin, insulin and digoxin.
