Forty-five year old gentleman who is seen in the outpatient infectious disease department following a recent hospitalization for a scrotal infection. His past medical history includes a deep venous thrombosis. His medications included intravenous antibiotics and diphenhydramine.

Clinical History 15
Fifty-two year old gentleman with coronary artery disease who presented to the hospital with an accelerating pattern of angina pectoris. Co-morbid conditions include hyperlipidemia, hypertension, peptic ulcer disease and chronic anxiety. Medications at the time of this electrocardiogram included aspirin, simvastatin, cholestyramine, a multi-vitamin, metoprolol and topical nitroglycerin.
