Sixty-five year old woman with a history of hypertension and chronic obstructive pulmonary disease who presents to the emergency room with a recent history of generalized weakness and exertional dyspnea. Her medications included inhalers, lisinopril and furosemide. By history she has not suffered a myocardial infarction.
