Clinical History 36
Sixty-one year old gentleman with ischemic heart disease, severe left ventricular systolic dysfunction and prior coronary artery bypass graft surgery who presents for cardiology clinic follow-up. Co-morbid conditions include severe peripheral vascular disease and hyperlipidemia. His medications included warfarin, lisinopril, digoxin and furosemide.
