Eighty year old woman with diabetes, hypertension and advanced peripheral vascular disease who presented acutely to the hospital with a gangrenous right foot. Medications at the time of this electrocardiogram included insulin, topical nitroglycerin, captopril.furosemide, amiodarone and metolazone.
