Fifty year old woman with diabetes, hypertension and advanced peripheral vascular disease

Eighty year old woman with diabetes, hypertension and advanced peripheral vascular disease who presented acutely to the hospital with a gangrenous right foot. Medications at the time of this electrocardiogram included insulin, topical nitroglycerin, captopril.furosemide, amiodarone and metolazone.
