Forty-six year old man with coronary artery disease status post an extensive anterolateral myocardial infarction six years prior to this electrocardiogram who returns for preoperative evaluation priorto implantable cardiac defibrillator generator replacement. His medications included metoprolol, simvastatin and aspirin.
