Seventy-seven year old gentleman admitted with a one week history of orthopnea and progressive dyspnea upon exertion. His past medical history includes long-standing non-insulin requiring diabetes mellitus, hypertension and chronic renal insufficiency. Medications at the time of this electrocardiogram included furosemide, aspirin, topical nitroglycerin and glucotrol.
