Thirty-nine year old gentleman admitted with a two day history of chest discomfort, worse with inspiration and a supine position, relieved with leaning forward. The patient was treated with oral nonsteroidal anti-inflammatory agents with symptomatic improvement. A myocardial infarction was excluded by serial cardiac enzyme analysis.
