Twenty-six year old gentleman with severe rheumatic aortic insufficiency seen in preoperative cardiac evaluation prior to anticipated aortic valve replacement. A recent echocardiogram demonstrated severe aortic insufficiency, a dilated left ventricle with mildly reduced left ventricular systolic function and left ventricular hypertrophy.
