Eighty year old gentleman with coronary artery disease and severe left ventricular systolic dysfunction admitted for evaluation of sustained ventricular dysrhythmias. His past history includes chronic renal insufficiency, congestive heart failure and severe mitral insufficiency. His medications included amiodarone.furosemide and digoxin.
