Seventy year old gentleman with coronary artery disease and moderately severe left ventricular systolic dysfunction who is status post coronary artery bypass graft surgery in the remote past. He returns for cardiology and pacemaker follow-up. He also has a history of ventricular tachycardia and is status post implantable cardiac defibrillator placement. His medications at the time of this electrocardiogram included captopril, aspirin, metoprolol, furosemide, warfarin and topical nitroglycerin.
