Sixty-one year old gentleman with coronary artery disease and a myocardial infarction six years priorto this electrocardiogram who returns for outpatient internal medicine follow-up. Co-morbid conditions include hypertension, insulin requiring diabetes mellitus and carotid vascular disease. His medications included captopril, isosorbide dinitrate, insulin, aspirin, digoxin and furosemide.
