Sixty-three year old gentleman with progressive exertional angina pectoris who is referred for a cardiac catheterization. A recent thallium stress test demonstrated an inferolateral myocardial infarction with peri-infarction myocardial ischemia. His medications included isosorbide mononitrate. A cardiac catheterization demonstrated moderately severe left ventricular systolic dysfunction and three-vessel coronary artery disease.
